Please attach the following documents if available.

• Diplomatic note (Click here (PDF 13 KB) for sample of diplomatic note) that clearly states the nature and purpose of travel, the intended travel/departure dates, and the sponsoring organization or entity
• Any letters of invitation from the U.S. government or other sponsoring agency
• List of group members (including names, passport numbers, employers, and positions)
