IMPORTANT NOTICE
Beginning April 12, 2022, the U.S. Consulate General in Ho Chi Minh City will TURN AWAY immigrant visa (IV) applicants who have not brought ALL the required documents to their interview and have not uploaded ALL the required documents to CEAC. For further information, see here.
You are responsible for ensuring that all required documents are available at the time of the interview. Please verify you have all required documents to prevent multiple trips to the Consulate.
You must have both the original and a copy for the documents listed below. We will return the original documents after the interview. All documents not in English or Vietnamese must be accompanied by a certified English translation that includes a statement by the translator that the “Translation is accurate” and the “Translator is competent to translate.” You must arrange the copies of your documents in order as listed below and separate all the original documents from the copies. Each individual should have one set of required documents. Failure to present satisfactory documentation may delay or prevent issuance of your visa.
1. COVER SHEET: For cases processed through Consular Electronic Application Center (CEAC), please print out the COVER SHEET here and complete all the items listed in the COVER SHEET to make sure you are ready for your interview. You will be TURNED AWAY and requested to reschedule a new interview if the COVER SHEET have not been completed. You must make sure that 1) You have uploaded ALL the updated documents to CEAC; and 2) You bring to your interview ALL the ORIGINAL/CERTIFIED COPY documents that match with the documents you uploaded to CEAC. All applicants who have not completed their medical examination and brought their information sheet from the panel physician will also be turned away.
2. Address Registration: Please visit ustraveldocs NOW to create an account, register your address, and print out the Address Registration Confirmation Page. You MUST bring a copy of this Confirmation Page with you to the interview and whenever you are scheduled to come to the U.S. Consulate after your interview. Failure to submit this Confirmation Page will delay the visa processing and visa delivery. Please note that each beneficiary with a different case number must follow the instructions to create their own account and register their mailing address separately.
This requirement is not applicable to K (Fiancé), Refugee/Asylee and Amerasian visa applicants.
3. Appointment Letter and Confirmation Page (with barcode) of Form DS-260 [for Immigrant Visa (IV) applicants] or Form DS-160 [for K Nonimmigrant visa applicants]: Upon your arrival to our office, please present the Appointment Letter and the confirmation page (with barcode) of Form DS-260 [for Immigrant Visa (IV) applicants] or Form DS-160 [for K Nonimmigrant visa applicants] to the guard for entrance and to our consular staff to get your queuing ticket. Please staple the DS-260/DS-160 Confirmation Page of the principal applicant (with barcode) under the Appointment Letter.
4. Visa Photos: Two (2) full face color photos for each applicant taken within 6 months. Write the applicant’s name and date of birth on the back of each photo. Please visit the Department of State’s website for more information on visa photo requirements.
5. Identification (ID) Card: Copy and original.
6. Passport(s): Copy and original. The passport(s) should be valid for at least 60 days beyond the validity of the visa. One (1) copy of the passport biodata page for each applicant, including children listed in their parents’ passport. If you have new blue-covered passport(s), you must have the passport(s) endorsed with place of birth. Further details are available on our website.
7. Household Registration Book: Copy and original.
8. Birth Certificates: Copy and original birth certificates of the petitioner, each person listed on the application, and all children of the principal applicant (even if he or she is not accompanying). In case you do not have the petitioner’s birth certificate, the officer will review the case and provide guidance at the interview. If you or any children were adopted, you must submit an official adoption decree.
9. Marital Status Documents (not required if never married):
* Marriage certificate if you are married (copy and original).
* Legal termination of any prior marriages of the petitioner and the beneficiaries. (Copy and original of divorce decree or death certificate of ex-spouse, if applicable).
10. Original Justice Record Check #2: Visa applicants aged 16 years or older must have a valid justice record check #2 issued within the past two year by Department of Justice office in the district where you currently reside or at your official residence as registered in your household registration book (Ho Khau). 11. Original Foreign Police Certificates (if applicable):
* IV applicants: Original foreign police certificate(s) from any country in which you resided for at least twelve months since attaining the age of 16. For further information, please click here.
* K applicants: Original foreign police certificate(s) from any country in which you resided for at least six months since attaining the age of 16. For further information, please click here.
12. Court and Prison Records (if applicable): Copy and original. Persons who have been convicted of a crime must obtain a certified copy of each court record and of any prison record, regardless of the fact that they may have benefited subsequently from an amnesty, pardon, or other act of clemency. Court records should include complete information regarding the circumstances surrounding the crime of which the applicant was convicted, and the disposition of the case, including sentence or other penalty or fine imposed.
13. Military Record (if applicable): Persons who have served in the military forces of any country must obtain one copy of their military record. For further information, please click here.
14. Medical examination results: Please submit medical examination results or the Information Sheet provided by the panel physician as proof of medical examination completion. If the physician gives you these results in a sealed envelope, PLEASE DO NOT OPEN THIS ENVELOPE. Instead, bring it to your visa interview. Please note that the examining physician will discuss the medical results and any required follow-up treatment with you. You must perform any follow up treatment at the Panel Physician’s office. 15. Evidence of Support:
* For IV applicants: The petitioner and any joint sponsor must submit a complete and signed Form I-864, Affidavit of Support for each eligible travelling applicant. A photocopy of the petitioner and joint sponsor’s IRS tax transcripts and any relevant W-2s must be included. For further information about required financial documents, please click here.
If an I-864 is submitted by a joint sponsor, he/she must submit proof of his/her legal status such as a copy of his/her U.S. Birth Certificate, U.S. Passport, Naturalization Certificate, or Lawful Permanent Resident Card.
If the evidence of support (I-864) has already been submitted to the National Visa Center, you do not need to submit it again at the interview. If not, please be prepared to submit it on your interview date. In some cases, if the evidence of support in file does not meet the guideline qualification, you will be requested to submit other financial documents after your interview.
Photocopies and scanned versions of I-864s and other associated documents are acceptable.
* For K applicants: The petitioner may complete and sign Form I-134, Declaration of Financial Support for the principal applicant and one copy for each eligible travelling applicant. A photocopy of the petitioner’s IRS tax transcripts and any relevant W-2s must be included.
When applicable, joint sponsor should include all of the documents listed above, plus proof of immigration status such as a photocopy of his/her U.S. Birth Certificate, U.S. Passport, Naturalization Certificate, or Lawful Permanent Resident Card. The officer will make the decision whether a joint sponsor is accepted at the interview.
16. Evidence of Relationship: You should prepare to present relevant documentation to establish the bona-fides of your relationship with the petitioner and with your derivative family members (if any).
* Fiancé cases (K): Please label and itemize all materials in chronological order and separate them into two groups: before and after engagement. Evidence may include but is not limited to photos, correspondence, phone bills, and other relevant items to assist in demonstrating to the consular officer the credibility of the relationship with the petitioner. Please note that you should remove your photos from albums and bring your loose photos only.
* Spousal cases: Please label and itemize all materials in chronological order and separate them into two groups: before and after marriage. Evidence may include, but is not limited to photos, correspondence, phone bills, and other relevant items to assist in demonstrating to the consular officer the credibility of the relationship with the petitioner. Please note that you should remove your photos from albums and bring your loose photos only.
* If you are the petitioner’s stepchild: The original marriage certificate of the petitioner and your biological parent and a photocopy along with divorce records for any previous marriages of either parent.
* Employment based petitions: Your employer in the U.S. must provide a written statement confirming that the employment initially offered to you is still available. The statement should be on the stationery of the employer’s business organization and must be originally signed and notarized.
* Other cases: Old household registration book (ho khau), old school transcripts (hoc ba), old photos, hospital birth records, baptism book, etc. which support the claim of a petitionable relationship.
