Thank you for your interest in requesting a Consular Report of Birth (CRBA) interview appointment at either the U.S. Embassy in Hanoi or the U.S. Consulate General in Ho Chi Minh City. Please note that this form should only be used if you have already completed your CRBA application online (eCRBA). Please follow the instructions HERE before returning to this CRBA Appointment Request Form.

You have successfully submitted your CRBA Appointment Request. We process requests in the order of receipt and will make every effort to respond to your request as quickly as possible. Please do not send your request more than once as duplicate requests will delay our response time.
