Apply for a Passport

The U.S Citizen Services unit issues emergency U.S. passports and renews passports as necessary.  Information on U.S. passports and fees is available on the State Department’s website. If you are applying for a passport, you will need to provide a passport photo that meets our requirements. All consular fees are payable in U.S. Dollars or Vietnamese Dong in cash, or with the following credit cards in U.S. Dollars: Visa, MasterCard, Discover and American Express. We do not accept checks.

To apply for a routine passport service, please schedule an appointment.  Appointments for U.S. Citizen Services are free.  Please contact us if you would like to report fraudulent activity related to our services.  If you need to request an expedited appointment, you must first schedule an appointment online and then submit an Expedited Request.  When you submit your expedite request, you must include the reason for the expedite request, the date and time of your scheduled appointment, and provide documentation to support your claim.  The American Citizen Services (ACS) Unit will consider your request.

We forward passport applications to the National Passport Center in the U.S. The National Passport Center then prints the passport and sends it back to us. This process takes approximately two weeks. We strongly encourage you to apply for a passport well before any planned travel.  You can check on the status of your passport application here.

If this is your first passport, you must …

Adults (age 16 and older)

  • Make an appointment and print out your confirmation sheet.
  • Apply in person.
  • Present evidence of U.S. citizenship  [e.g. Original/Certified U.S. Birth Certificate, Consular Report of Birth Abroad (FS-240) or Certification of Birth (FS-1350), Certificate of Naturalization, Certificate of Citizenship].
  • Present one valid and original identification (e.g. Foreign passport, U.S. Driver’s License, U.S. Military/State/ US Government ID, Green card, etc.).
  • Provide one color passport photo  (2”x 2” or 5cm x 5cm) with a white background taken within the last 6 months.
  • Complete form DS-11.  Overseas applicants may complete the form online but cannot submit it electronically. Please PRINT the completed form and bring it to the appointment.
  • Bring cash or a credit card to pay the fee. All fees are payable in U.S. Dollars or Vietnamese Dong in cash. In addition, fees are also payable with the following credit cards in U.S. dollars: Visa, MasterCard, Discover and American Express.  We do not accept checks.
  • A return delivery courier slip (available at the Embassy/Consulate) filled out by you will be used to return your passport.  Please note that you will pay for this service in cash directly to the EMS courier service.

Children Under 16

  • Make an appointment and printout your appointment confirmation.
  • Children must appear in person accompanied by both parents.
  • If either parent cannot be present, the appearing parent will need to provide either a photocopy of the non-applying parent’s ID and a notarized form DS-3053 Statement of Consent (PDF-41KB) (Note: All DS-3053s for parents present in Vietnam must be notarized by a U.S. Consular Officer) or form DS-5525 Statement of Exigent/Special Family (PDF-40KB) Circumstances, depending on your circumstances.
  • Present evidence of U.S. citizenship (e.g. Original/Certified U.S. Birth Certificate, Consular Report of Birth Abroad (FS-240) or Certification of Birth (FS-1350), Certificate of Naturalization, Certificate of Citizenship.
  • Parents’ valid ID.
  • Present proof of parent-child relationship (e.g. Minor’s U.S. Birth Certificate/ Minor’s Consular Report of Birth Abroad/Minor’s Foreign Birth Certificate / Adoption decree with adopting parents’ names/ Court order establishing custody or guardianship / Judicial declaration of incompetence of non-applying parent/Death certificate of non-applying parent.
  • Provide one color passport photo (2” x2” or 5cm x 5cm) with a white background taken within the last 6 months.
  • Complete form DS-11.  Overseas applicants may complete the form online but cannot submit it electronically. Please PRINT the completed form and bring it to the appointment.

If you are renewing your passport, you must …

For adult (Age 16 or older)

  • Make an appointment and print out your confirmation sheet.
  • Apply in person.
  • Present your most recent (current or expired) U.S. passport.
  • Provide one color passport photo (2”x 2” or 5 cm x5 cm) with a white background taken within the last 6 months.
  • Complete form DS-82 (if your most recent full validity passport was issued within the last 15 years and when you were age 16 or over, and the passport you are submitting is not damaged). Otherwise, complete form DS-11.  Overseas applicants may complete the form online but cannot submit it electronically.  Please PRINT the completed form and bring it to the appointment.  Please note that the electronic form lists DS-11 on the top left of the first page of the application form, but will change to the accurate form, i.e a DS-82, if you provide the requisite information.  Please enter all required information completely and accurately.
  • Bring cash or credit to pay the renewal fee. All fees are payable in U.S. Dollars or Vietnamese Dong in cash. In addition, fees are also payable with the following credit cards in U.S. dollars: Visa, MasterCard, Discover and American Express. We cannot accept checks.
  • A return delivery courier slip (available at the Embassy/Consulate) filled out by you will be used to return your passport. Please note that you will pay for this service in cash directly to the EMS courier service.

For child (under 16)

  • Make an appointment and print out your confirmation sheet.
  • All applicants (children) must appear in person accompanied by both parents.
  • Submit current or expired passport.
  • Present proof of parent-child relationship (such as Minor’s U.S. Birth Certificate/Consular Report of Birth Abroad, Foreign Birth Certificate, Adoption decree, Court order establishing custody or guardianship/Judicial declaration of incompetence of non-applying parent, Death Certificate of non-applying parent)
  • Present parents’ valid IDs.
  • Provide one color passport photo (2” x 2” or 5cm x 5cm) with a white background taken within the last 6 months.
  • Complete Form DS-11.  Overseas applicants may complete the form online but cannot submit it electronically.  Please PRINT the completed form and bring it to the appointment.
  • If either parent cannot be present, the appearing parent will need to provide either a photocopy of the non-applying parent’s ID and a notarized form DS-3053 Statement of Consent (PDF-41KB) (Note: All DS-3053s for parents present in Vietnam must be notarized by a U.S. Consular Officer), or form DS-5525 Statement of Exigent/Special Family Circumstances (PDF-40KB), or primary evidence of sole authority to apply for the child, depending on your circumstances.
  • A progression of photos showing the changes of child’s appearance since the last passport was issued. Progression photos should establish a clear likeness between the applicant’s current appearance and the photograph used to obtain the previous passport or proof of citizenship.
  • Bring cash or credit to pay the renewal fee. All fees are payable in U.S. Dollars or Vietnamese Dong in cash. In addition, fees are also payable with the following credit cards in U.S. dollars: Visa, MasterCard, Discover and American Express. We cannot accept checks.
  • A return delivery courier slip (available at the Embassy/Consulate) filled out by you will be used to return your passport. Please note that you will pay for this service in cash directly to the EMS courier service.

If your passport is lost or stolen, you are advised to report the loss/theft to the local Vietnamese police in the area in which it occurred. Please schedule an appointment and bring the required documents to replace your lost/stolen passport. Passports reported lost or stolen are invalidated and can no longer be used for travel.

If you have urgent travel plans and your passport has been lost, stolen or is expired, we can assist you by issuing an emergency passport.  Such passports will be limited in validity, and cannot be extended (validity depends on the case but maximum validity is up to one year).

Passport holders are required to exchange their limited validity passport for a full-validity photo-digitized passport upon completion of their emergency travel, either through passport facilities in the U.S. or at U.S. Embassies and Consulate abroad.

Important! With the exception of emergency services, all U.S. citizen services must be scheduled in advance.

Adults (age 16 and older)

  • By appointment only. Printout the appointment confirmation.
  • All applicants must apply in person.
  • Police report of lost or stolen (with translation). The police report is needed to exit Vietnam and to replace your Vietnamese entry stamp by the Immigration Department, in addition to being important to any insurance claims you might make.
  • Present proof of U.S. Citizenship (if circumstances allow) such as a U.S. birth certificate, Naturalization Certificate, or prior U.S. passport or a copy of the lost/stolen passport (if available)
  • Present one valid form of photo identification such as driver’s license, foreign passport, state/military ID, etc.  (if available)
  • Provide one color passport photo (2” x 2” or 5cm x 5cm) with a white background taken within the last 6 months.
  • Complete Form DS-11 and form DS-64 (PDF-50KB).  Overseas applicants may complete the form online but cannot submit it electronically.  Please PRINT the completed form and bring it to the appointment.
  • Bring cash or credit to pay the passport fee. All fees are payable in U.S. Dollars or Vietnamese Dong in cash. In addition, fees are also payable with the following credit cards in U.S. dollars: Visa, MasterCard, Discover and American Express. We cannot accept checks.
  •  A return delivery courier slip (available at the Embassy/Consulate) filled out by you will be used to return your passport.  Please note that you will pay for this service in cash directly to the EMS courier service.

Children (under 16)

If you have changed your name or need to correct data on your passport, you will need to have this change reflected in your passport. Please note that passports can no longer be amended; therefore you will need to apply for a new passport.

Name Changes

If you want to have your passport corrected to reflect a name change you may receive a free replacement passport if the requested change is made within one year from the date of the original passport issuance. 

Documents needed to exchange your passport due to a name change:

  • Make an appointment and print out your confirmation sheet.
  • Your valid passport.
  • Completed form DS-5504.  Overseas applicants may complete the form online but cannot submit it electronically.  Please PRINT the completed form and bring it to the appointment.  Please note that the electronic form lists DS-11 on the top left of the first page of the application form, but will change to the accurate form, i.e a DS-5504, if you provide the requisite information.  Please enter all required information completely and accurately.
  • If your name changed, please submit evidence to support your name change such as an original/certified marriage certificate or divorce decree or court order, amended birth certificate, etc.
  • Provide one color passport photo (2”x2” or 5cmx5cm) with a white background taken within the last 6 months.
  • If applying for children under 16, a parent should sign the application form on behalf of the child. The personal appearance of the minor and the two-parent consent are not required.
  • If the change is more than one year after issuance, please follow the procedures for passport renewal and include Documentation showing name change (e.g. original marriage certificate, divorce decree or court order, amended birth certificate, etc.).
  • If you do not have one of the documents listed above, you can present (in person) three of the following (one of which must have been issued more than five years ago):
  1. U.S. state driver’s license;
  2. Vietnamese photo identification;
  3. Foreign passport;
  4. Additional official picture identification
  • Bring cash or credit to pay the passport fee. All fees are payable in U.S. Dollars or Vietnamese Dong in cash. In addition, fees are also payable with the following credit cards in U.S. dollars: Visa, MasterCard, Discover and American Express. We cannot accept checks.
  • A return delivery slip (available at the U.S. Embassy/Consulate General) filled out by you will be used to return your passport.  Please note that you will pay for this service in cash directly to the EMS courier service.

Data Correction (Printing Error)

If you want to have your biographical data corrected in your passport due to Department error, you may receive a free replacement passport with corrected data at any time.  To have your biographic data corrected, do the following:

  • Fill out passport application DS-5504.  Overseas applicants may complete the form online but cannot submit it electronically. Please PRINT the completed form and bring it to the appointment.
  • No additional fees.
  • Your passport with the incorrect biographical data.
  • If there is a printing error in the descriptive data, please submit evidence showing the correct information such as an original or certified birth certificate or a marriage certificate.
  • One color passport photo (2″ x 2″ or 5 cm x 5 cm) with a white background taken within the last six months.
  • A return delivery slip (available at the U.S. Embassy/Consulate General) filled out by you will be used to return your passport.  Please note that you will pay for this service in cash directly to the EMS courier service.
