Change Name or Correct Data on U.S. Passport

You will need to schedule an appointment online by following the steps on our Appointment System and submit your passport application at either the U.S. Embassy in Hanoi or the U.S. Consulate General in Ho Chi Minh City.  Once you have scheduled an appointment, please print out the confirmation page for access to our office on the appointment date.  If there are no slots available in the Appointment System, please go back to the system at a later time.  Appointment slots are opened approximately two weeks in advance, and additional slots are often added, so we recommend you check the system periodically for availability.

If you want to have your passport corrected to reflect a name change you may receive a free replacement passport if the requested change is made within one year from the date of the original passport issuance. 

Documents needed to exchange your passport due to a name change:

  1. Appear in person.
  2. Present your valid passport.
  3. Complete Form DS-5504 online here and PRINT OUT the completed form. Please note that the electronic form lists DS-11 on the top left of the first page of the application form, but will change to the accurate form, i.e a DS-5504, if you provide the requisite information.  Please enter all required information completely and accurately.  Note: you cannot currently list a “Country” when completing the “Emergency Contact” section on our forms.  Please list an emergency contact in the United States.
  4. Submit evidence to support your name change such as an original/certified marriage certificate or divorce decree or court order, amended birth certificate, etc.
  5. Provide one color passport photo  (2”x 2” or 5cm x 5cm) with a white background taken within the last 6 months.  Photo requirements are listed here.  You can also click here for a reference list of some photo shops located near our offices.
  6. If applying for children under 16, a parent should sign the application form on behalf of the child. The personal appearance of the minor and the two-parent consent are not required.
  7. If the change is more than one year after issuance, please follow the procedures for passport renewal and include Documentation showing name change (e.g. original marriage certificate, divorce decree or court order, amended birth certificate, etc.).
  8. If you do not have one of the documents listed above, you can present (in person) three of the following (one of which must have been issued more than five years ago): 1) U.S. state driver’s license; 2) Vietnamese photo identification; 3) Foreign passport; 4) Additional official picture identification.
  9. Bring cash or a credit card/ debit cards to pay the fee. All fees are payable in U.S. Dollars or Vietnamese Dong in cash, or in U.S. dollars with the following credit cards/ debit cards: Visa, MasterCard, Discover and American Express.  We do not accept checks.

If you want to have your biographical data corrected in your passport due to Department error, you may receive a free replacement passport with corrected data at any time.  To have your biographic data corrected, do the following:

  1. Appear in person.
  2. No additional fees.
  3. Present your valid passport with the incorrect biographical data.
  4. Complete Form DS-5504 online here and PRINT OUT the completed form. Please note that the electronic form lists DS-11 on the top left of the first page of the application form, but will change to the accurate form, i.e a DS-5504, if you provide the requisite information.  Please enter all required information completely and accurately.  Note: you cannot currently list a “Country” when completing the “Emergency Contact” section on our forms.  Please list an emergency contact in the United States.
  5. If there is a printing error in the descriptive data, please submit evidence showing the correct information such as an original or certified birth certificate or a marriage certificate.

When you arrive at our office, you will be served as soon as possible.  If your application is approved, you will need to fill out a return delivery courier slip (this slip will be available at our office) which will be used to deliver your passport by post (please note that you will pay for this delivery service in cash directly to the EMS courier service after your appointment).  We will then forward your passport application to the National Passport Center in the United States.  The National Passport Center then prints the passport and sends it back to us. This process takes approximately two weeks. We strongly encourage you to apply for a passport well before any planned travel.  You can check on the status of your passport application here.  If it has been more than two weeks since the application date and you have not received any notification, you are welcome to contact us here.  In your inquiry, please state clearly that you have waited more than two weeks from the passport application date.

If your passport application is refused, you will receive a refusal letter instructing you on how to continue the processing of your application.

Appointments for American Citizen Services are free (no cost).  If you wish to report fraudulent activity related to any of the Consular services we provide, please contact our Consular Fraud Prevention Unit atHanoiFPU@state.gov (for Hanoi) or HCMCFPU@state.gov (for Ho Chi Minh City).

In order to be fair to applicants, our office only consider expedite requests when there is an urgent need to travel due to an extreme emergency.  If you need to request an expedited appointment, you must first schedule an appointment online.  After successfully scheduling an appointment, you must visit our Inquiry Form to submit an expedite request.  Please include:

1) the reason your case is an extreme emergency and you must have an earlier appointment,

2) the date and time of your scheduled appointment and the date and time of your preferred appointment if your request is approved, and

3) attach evidence of your emergency situation or an explanation why an earlier appointment will help you resolve your emergency situation.

If you do not provide ALL the information listed above, your request will not be considered.
